Forms to be filled out only by the International Students:

• Other Visa Holders (F1,H1,J1 etc)
• Permanent Residents
• W-9 for NRA (Non Resident Aliens)

For those who do not have a Social Security Number at present:

• Application for Social Security Number 

         (The application form is in the 5th page)

Students waiting for a Social Security Number should fill out this form, in the interim:

• Affidavit of Compliance
• More Information on Social Security

Please bring the completed forms (in black ink only) to EIT business office at 2125K, J.M.Patterson. You can either submit them to Madhu or Katie. Please make sure to bring the following documents as well:

• Your Driver's License
• Your Social Security Card (A scan will do for US citizens. International Students should bring the original)
• If you do not have a Driver's License, you can bring your School ID and passport (again, a scan will do for US citizens)
• International Students should bring their Passport, Visa, I-94 and I-20 and also apply for a Social Security Card if they do not already have one.
